Multiple weak links in the erx chain.
Last one is at the pharmacy - pharm tech usally means under-educated, under-paid, uninterested youngish person. We are counting that they must 100% click on the right button, not delete our rx, in some cases translate from erx fax to paper or whatever system they have, and generally care about their job. Same goes when generating refill requests, I'm sure it's why i get multiple duplicate refill request over minutes or hours.
