I am having the same problem on my 2012 report. All items are fine until CQM Alternate, where the only populated columns are "Exclusions", which show 0s, and "P/F", which show red crosses. Other core, menu, and CQM Required are ok. In case it was a processing time problem as mentioned above, I rebooted both the server and the client, shut down all other clients, tried running the wizard from the server, and tried running 3 or 6 months, all to no avail. Mark, how do I find the "bad dataset in the program somewhere" that you speak of?
