I have the same issues with the pronouns and simple words. Also for some reason DNS will add the suffix ed or an s to the end of some verbs such as discontinue (d) or (s)...don't know why. I still think it saves me considerable time compared to typing. The transcriptionist needed proofreading too way back when I used one.
I guess you are dictating an impression or plan when you are switching from the note to the reports in the imported items folder??? You could open a blank note from the start button on the lower bar of your desktop, dicatate your note onto that and then copy and paste it into the AC encounter. ( That is the way I do my operative notes.)
