office note missing ( we saw the pt ) and it was billed . We can't find the note. The pt is on our schedule when we check .
Anyone else have this happen?

I am usually able to find the chart in the deleted section of my messages or in the deleted messages of the MA that triaged the patient. Unfortunately, that chart does not have any of my notes or prescriptions saved but can be completed if found.
What biling service? If billed in ACPM that would really be a mystery.
I have an idea of what might have happened as I have had similar. I say similar because of my question about billing. What I have found, is that should the chart inadvertently been closed or AC restarted the chart can be lost. If the restart happened while the chart was in ANY TAB OTHER THAN THE "Most Recent Encounter" the program is not able to bring that chart back up on restart.
