For our ACO we need 3 quality measures reported electronically going forwards.
#001 (CMS122): DM- A1c poor control >9% #134 (CMS2) : Preventive care and screening, Screening for Depression and f/u plan #236 (SMS165): Controlling High BP
DM and BP reports can be run thru reporting tab of AC. But not depression screen report.
In the previous versions there was PHQ9 screen button in the right lower quadrant. It was working good. Then it got unusable and would not work, when we called AC they said they were aware of the nonworking issue, then it is gone in the newest versions.
So, we have been doing PHQ9 screen on paper. But now I believe it cannot be captured for electronic reporting.
Hopefully we can get that PHQ9 button back fast as our payments are based on reporting.
That is odd, but I think I know the problem. The PHQ9 button is still available but only for patients that have not had a screening in the last year. Are you sure these are people that have not had one in the past year? I think his will meet the MIPS guidelines.
If you are looking to do the PHQ9 more frequently then you can start a second testing by going to the "summary Sheet" tab, then bottom left "Decision Support & Injections" tab, "Complete" tab where you should see the last PHQ9 testing date. Double click on the PHQ9 name box and the "Clinical Quality Measures" window will open. Go to the "Screenings & Tests" tab and there you can redo the PHQ9 via the blue hyperlink. You can also see a PDF of the last test performed via the link at the bottom.
Hope this helps. I think there is an easier way to get to the Clinical Quality Measures but I can't remember right now. This reads as being complicated but it really isn't just follow the steps. If you have problems let us know, if it works for you then let us know!/
Thanks for your effort. But it is not answering my question.
Per our instructions from ACO, we are supposed to do this for every patient once a year. So, today I am seeing a new patient, never seen before and that PHQ screen button is not showing up on AC 12.0.0. build 369.
If that PHQ-9 button on the most recent encounter tab (just like BP, pulse, RR fields) showed up for every patient, every visit it would be nice for various reasons.
Then in the end the score would be calculated electronically.
The path you showed I believe does not do that. It is too convoluted and more to see if it was done. I think a better way is to have PHQ-9 button available and let the provider decide if they want to do it. If they do it, then it is scored and that helps.
If you need more clarification about this issue, let me know.
Thanks for the feedback, not sure why it is not showing up on new patients. The button for the PHQ-9 does show up on my new patients. I tested this morning. It shows up under the clinical assessment button on the bottom right. I attached a screen shot for you of the button and the window it brought up for the PHQ-9 questionnaire. You might check with tech support and see if this is just a setting in your system. I looked under user preferences but did not see where this could be changed.
I also went to the "Summary Sheet" tab and under "Decision Support & Injections" the PHQ-9 screening test was listed and all I had to do was click on it for the testing box to open.
You are correct in that what I sent was to see if the PHQ-9 was done in the past and if you wanted to do another one sooner than one year this is an option to retest. I think there is an easier way but can not find that route at this time.
I am still not sure if the results would be a report in AC. If you are currently being able to report for DM and BP but I would suspect once in AC the information for PHQ-9 would be able to be reported out.
Once you answer the 10 questions, click the check mark at the bottom, does the PHQ-9 score flow into that particular note in your system?
In the past it used to flow, the PHQ-9 score is so-so, into the progress note.
I was able to make it appear on progress note. You have to go to decision support and check yes to all grade recommendations. I am not sure which grade it is under, but checking yes to all grade recommendations made it appear on the most recent encounter. Photo enclosed.
But, on the most recent encounter, once you open the clinical decision tool, answer the questionnaire and click the check mark, the score does not flow into the note. So there is a bug in the AC that the tech support said was aware, but then that was many versions back and is still not fixed, so it is non-usable and non-working part (as I mentioned earlier).

Glad you got it to appear in the progress note but sounds like it is intermittent? Am I understanding correctly? I have seen this happen on a few patients. The Score will not show up as being answered in the chart note under plan but does show up under the imported tab. It will also show up under the Summary tab. The sample I just did does have the PHQ-9 in the chart plan.
2. These seem like different issues than what you originally posted where the button was not present in the chart. Is that correct?

Top right in Yellow, Decision Support Box, left click, big box opens with Clinical Quality Measures, tabs across the top, Screenings and Tests, left click, search PHQ-9 and it will open the PHQ-9 box from here click the clinical assessment tool and you can do the test. The URL for the test is in blue. It shows at the bottom of the page the last time a PHQ-9 was performed.
