How about being able to create or import our own forms for permission, privacy, or what have you combined with the ability to capture the patient's signature on our tablets??? This way we can do away with some more paper, save it in a "forms" folder in the imported items section of the chart, and be able to print a copy if needed or if the patient requests a copy of their forms (some do sometimes). This would be much better than having the patients sign a hard copy and scanning it in later. Being able to customize these to fit our own states or needs would be best. I could see having a plastic covered hard copy for the patients to read, and then having them sign the tablet once or twice on the proper forms. Any other suggestions??? :idea: :?:

Hocky, I think this is an excellent idea. I've been meaning to post such an idea myself, but you beat me to it.
We also have a screening questionaire that we would like to find a way to integrate into the system w/o having to have to scan it. Its a mood disorder questionaire. We'd like to be able to somehow have the patient fill it out electronically and then save/import it into the chart.

Hey But I want to capture the all important "signature" that says yes you read it!!! Your idea might be able to intergrate into that new service Jon is offering for the health questionaire??? Not sure how much you can customize it from patient to patient, but that would be a good point for that as well...I'm thinking about a well built cabinet like a video arcade game so the computer and the screen are enclosed and protected, perhaps with a bar stool in front of it if we were to use that new health history thing. Don't need some butter fingers dropping an expensive laptop or spilling soda on it and having the "china syndrome" happen. :idea:

I agree w/ all the signature stuff. In fact, I'd like to be able to customize a built-in form that they sign on-screen. Our website from Medem has a registration form that could be used. However, it can't be electronically xferred to ac (must be printed, signed, and scanned) and it does not have the verbage that they sign to allow us to directly bill their insurance. We could post HIPAA also, but again it still must be printed and signed (and scanned). It would be great to be able to have a customizable form (or 2)in AC that the patient could sign on a tablet pc. I think this is what you were suggesting. This would be automatically in their chart. So when they arrive for their first visit (or have a change of insurance or address) we can pull up the document, show it to them, have them sign it right there, a BAM! all done.

If you keep in exam rooms, you end up buying 1 for each room. I heard some doctors are using this. My 0.02$ worth. Regards.

Since we use Microsoft Office, we can import forms that we've made in Word and then they are editable. We do NOT do this however. We have patients sign these forms on the tablet pc at the reception desk, our receptionist "prints" it to a Microsoft Document Image file and then imports it into the patient chart. This makes what is a Word form a permanent chart form that can't be edited. . . needed for integrity of information. However, we do have a few forms that we really like to use and do use them outside Amazing Charts and then print to MDI and import into Amazing Charts.

I am also very interested in capturing patient's signature electronically for office forms including our privacy and financial policies. I am looking into the following.
Have adobe acrobat professional installed on tablet PC. Have your form for patient to sign in adobe acrobat pdf format. Use "review and comment" feature which allows you to write on form with "digital ink." Save it and import into amazingcharts.
