I would like to get rid of the express version of SQL and use the full capabilities of the database. Is there a way to migrate this without causing problems with updates in the future. All of the features and especially the meaningful use calculation would be much faster if I could use all the power of my server which is severely limited using express SQL.

RRHC,
Unless you are a fairly large organization with many employees, SQL Express is not your problem for the performance of AC. And, AC will not support its software on SQL Server Standard or Enterprise.
The only differences performance-wise between SQL Express and Standard are the amount of RAM they can use (Express is limited to 1GB regardless of how much you installed on the machine) and number of procoessors (Express is limited to 1 regardless of how many are installed).
If you are a single provider practice with 1 - 5 users all the way to a 5 - 7 provider practice with two dozen employees, SQL Express should be all you need.
If you are experiencing slowness, then it is time to look at your server and network.
Also, be aware that SQL Server 2012 Standard is $800 for the server and $120 per client access license if you do Server + license. If you do per-processor core licensing, it's $7,000 for the first four processor cores of the machine.
Before spending that kind of cash, you really should check your network first.

My problem is the timing out of meaningful use data mainly. My server is brand new HP with 8 core server processor, RAID 10 solid state hard drives (8 drives installed) and 16 gigs of memory and Windows SBS 2011. I get good quarterly results, but if I run a full year the last 3 CQM measures report all zeroes indicating a time out problem. I sent my database to AC and they found no problems with the data and they were able to run the full year MU with no problem. It's very frustrating to have a 1000 horsepower server and only use a fraction of it to crunch numbers. And yes, I have tried running it directly on the server with all other computers turned off so it was not doing anything else. I also have run multiple server speed tests and the thing is significantly faster than my home computer running an i7 with similar stats but not server software.
I guess I'll just have AC run my MU and fax me the results to do my attestation.

The issue must be elsewhere. I can run a full year with no problems. I have RAID 10 WD RE4 Hard Drives still slower than a single SSD, Xeon E3-1235, 16GB RAM.
I have a ton of people on RAID 1, same specs running multiple providers without issues.
What happens when you run it?

I would also state that it wouldn't be due to SQL Express. Also, AC has no other alternative that to use SQL Express as their SQL Engine. They can't really expect everyone to pay the kind of money they would need to to get SQL 2005. Plus, once they did and they moved to 2012, MANY practices would be caught with their pants down and not be ready to pay for the newer SQL.
As far as your question, uninstalling the instance, installing 2005, 2005 or 2012, you would then make the new instance. You would have to know how to do it plus know the sa password.

RHCC, If possible, could you make an autosig in your profile so we know you real name. It is hard to refer to you as RHCC. It may be a Rural Health Care Clinic. But, still difficult.

I was able to run a full year of MU for the other doctor in my office that has approximately as many patients as I do. Mine still gives 0's for the last 3 CQM measures if I run more than 8 months. I also runs each quarter with no problems for me, I was hoping to narrow it down to corrupted data in a quarter then keep narrowing it until I found the date that was causing the timeout. This was unsuccessful.
My server is HP ProLiant DL385 G7 6136 1P 8GB-R P410i/256 Hot Plug 1 SFF 460W PS Base Server Upgraded to SSD drives RAID 10 with 1TB of hard drive space actually available and they are the fastest SSD drives made. It was purchased 2 months ago. SBS 2011 and 16 gigs of top grade ECC RAM.
