I was looking at formedic and wondered if anyone has used it and if there is somesome way to use in in AC. Looks good and is free! http://www.formedicmhq.com/I wrote to the company, but have yet to hear from them. Thanks!

I would be happy if my patients simply completed the form we give them to bring in before their annual exams!! We can even call them 2-3 days before their apppointment and remind them to complete the form (we'll fax one to them if they lost it or ask them to stop in and pick up a new one) and bring it with them and I would say over 50% forget to bring it in or act like they have never heard about it. We even ask those who don't have it completed or new patients to come in early so they can complete it but very few comply. Same thing goes with getting diabetics or hypertensives to bring in their sugar/BP records. I can ask them time and time again to bring those in...."Oh, I forgot" or, "The dog must have eaten it".
The Formedic site looks great but I sincerely doubt I could ever get my patients to complete it prior to their visit.

When we were using IMH, we had pretty good success with getting patients to complete it in advance. It has to be part of your routine to ALWAYS mention it to patients that are scheduling appointments. Now, you won't get even close to 100%. But each one that does it can save you some time in the room and in completing the chart.

I like free. It looked useful for history gathering, another source of info from the patient. The third set od questions in the history taking process gets the best history. How does it work for anual exams or out patient consults...? Thanks for the link.
