Resign yourself to the fact that this will NEVER be fixed. It is not a bug. It is not on purpose. It is because the developers do not understand the fact that its being set up this way causes huge issues. I have been told it remembers THAT PARTICULAR script.
This one single issue causes me more frustration than any other issue on AC. I have complained about it since the first day this prescription writer came out. You are also a member of a very special club: Those who have an issue with this problem. And, it is a problem.
What I have told the developers is that there is absolutely no reason why any doctor would either want the first three checked or unchecked at different times. In fact, one would think the default would be for the first three to be checked. The latter two should default to unchecked. When you write your script if you want to do a DAW, you can. If you want to omit your signature you can. In fact, my guess would be that even the omit your signature would be something a doctor would want checked or not checked by default.
Many times, if I have not saved a particular script, I will see the following:

I then have to check the top three.
Once that script is saved on THAT chart (I believe -- I will never truly know), you will get the following when you refill it:

The strange thing is, since after doing the refills, my mouse pointer generally moves across the radio buttons, I always see it, and I check them. My nurse forgets (these are scripts which I sign after), and the patient is sent back to the office, because there is no DEA# (most pharmacies have it on file and don't care).
I know we always say, "This could be fixed with one line of code." This isn't even about code. It is about the developers understanding how we use the prescription writer and a general philosophy about what would be a preference.
The way to fix the problem would be to go to the same place in the admin section where you set your ePrescribe (with the twips, etc.) and allow you to choose what you want as the default. I would select the top three to be checked EVERY time. I see no reason not to have my license number and my NPI on each script. Some would argue you don't need the NPI, but what the hell. But, for them, don't select it as a default.
One workaround that I used to use, although it makes your letters and other letterhead-requiring documents look a little silly, is to put your DEA# in the second line of your address. In this way, it is on everything you send including your scripts.
