When pharmacies send us a request for refill of a prescription and we respond to it, it creates an addendum in the patient's medical record. This is congesting the view of past encounters and is not needed. AC records the date of refill in the medication history, so an addendum is not necessary. We do not manually do an addendum each time we refill a medication that a patient calls to refill. This is only happening when the pharmacy sends a refill request to us electronically and we accept or deny the rx and send it to the pharmacy. Each prescription refill has its own addendum, so if the Dr. refills 10 medications on a given day this way, AC creates 10 addendums. We would like a shut off button......
