After doing an encounter note a few days later when I have time, I alter the date to match the actual date of the patient's visit. When I go back to that chart and click on the most recent encounter, the date of that same note is listed as the date that I performed it, and not the date of the encounter. Is this intentional or are the bean counters at work again?

Can you explain this a little more? Are you aware that if you change the date of the encounter before you sign-off, it will reflect that change. Once you sign-off the note, the date cannot be changed.

I think in such situation you need to be extra careful.Your signed set the direction for further inquiries.The date could be an issue.
