I have just started using DNS and it transcribes as I dictate very well. However, when I sign the chart and go back to look at the deictation there is no text where there once was text on the history, etc. I tried this twice for the same patient with the same result. Interestingly, the problem list/diagnoses were still there. I assume I am doing something wrong. Any suggestions?

Sorry to hear about your troubles.

I have used DNS Preferred and DNS Medical 9.5 for months now, and have never experienced the problem you describe.

Do you only have this problem with text transcribed with DNS, or does it also occur with data entered via keyboard?

Note that diagnoses entered via the ICD-9 lookup will remain in the Problem List, even if you close a file without saving it.

Or are you saying that the data in the Assessment and Plan gets saved, but nothing else does?

Give me a little more detail, and I'll try to help.

Thanks for your offer of help. I have DNS 6.0 and just tried it yesterday for the first time. It transcribed beautifully, but the info on the history and physical is not saved except for one medication I entered manually.The dictated asessment and plan were also lost, as you noted the ICD-9 codes/problems were saved. However, I dictated the immunization part and it is saved, but saved the second time dictated. I even dictated the entire thing a second time thinking I had not saved it properly, but had the same result.
Any input is greatly appreciated. I am new to AC, only started the trial about 6 weeks ago so it may be "user error."

Sorry, but I'm still not clear on one question:

If you type in the whole note by hand, and then save the note, does it save okay?

Because it sounds like Amazing Charts is just not saving your notes for some reason.

All of the items which you are successfully "saving" are items which will you can change in your patient record without saving a note.

In other words, you can open a patient's chart and add ICD-9 diagnoses, Medications, and Vaccinations, then close it without saving, and those items will remain in the patient's Summary Sheet.

It makes me think the file is just not being saved at all. As you say you are new to AC, I will assume that you have never successfully saved a chart and point out that you have to do the following to save:

1. Patient Record: Click "Sign Chart" at the bottom right-hand corner. The Coding/Signing page will appear.

2. Coding/Signing Page: Click "Sign It". You may get some message boxes. If you click properly, a window will appear telling you that your note is being saved, and there will be a progress bar, and then it will tell you the file was successfully saved, and your records will be updated.

Yes, if the notes are typed in there is no problem. When using DNS, the note is transcribed as I speak and then signed. Although, I do not remember clicking the sign button now that you mention it. I have typed several notes in and saved them without difficulty in the past. Now after "playing with the settings" on DNS, it does not even transcribe my words. Must be a wrong setting, but have not had a chance to check it again. DNS is used parttime when not working my "real job" as a hospitalist. Thanks for your help and the link above.
