are you referring to Medicare/Medicaid CMS Meaningful Use Incentive? Or is "Texas EHR reimbursement program" something entirely different?

Every state has a different program with different requirements to get the Medicaid incentive. Some states have not even started their programs yet. Here in Maryland, the rules have not even been published. Sounds like you need to look into the state requirements first. The good news is that some states don't require nearly as much for the Medicaid incentive as Medicare does. For the first year, you may be able to simply attest that over 30% of your patients are Medicaid and that you are using a "Certified" program.

I am referring to the Medicare/Medicaid CMS meningful use incentive program for the state of Texas , they need me to upload the panel for three months showing patient mnedicaid number/dos/claim status/provider number. I started a small solo practice and dont use any PM , print the encounters on paper and mail them . When I asked the AC tech they said they cant help in this and i should ask my PM( which i dont have or use) , surely there should be a painfree way of getting this data and uploading it on the CMS website ,I am so hoping that a forum member or AC should be able to help /guide me with this . thanks

What you require should be done by exporting to billing that is currently in AC.
From the main page go the the billing menu and click on pull/export bills by date. Then put in the relevant dates and click on pull bills. This then gives you a new screen where you decide name and export location and what type of export (see below). Then hit the export button and voila, you are done. Text files are small, even for an entire year. It should be easy to then upload however you need to do this. Uploading I cannot help with, I do not know the TX system.
Again, every state is different. We did not have to do this. What format do they need? My suspicion is that simple text will be the correct one, since it is easily exported into excel and the like.
We use this for billing and export a file to MTBC. I also download it and do practice analysis through excel.
This should provide them with what they need, probably more than what they need.
Another way would be to ask your biller for an export file, but this is very quick and easy.
