How are people handling the following situation: a patient calls and would like B-12 pills. this is a simple telephone call and I prescribe this, and call her pharmacy. How do I best enter this encounter, showing only the conversation and the prescribed med ? I of course want the medication to appear in the meds list for future reference.
?Message: Pt had lab work from urgent care faxed over because she is concerned that B-12 and, and pt complains of leg numbness. Would you be able to prescribe B-12 shots? Pharm: xxx-xxx-xxxx. pt xxx-xxx-xxxx . BW
BLM: Her B12 level is only border line low, I?ve spoken to patient should do well with oral medication - I?ve called in : Vitamin B12 1000 mcg oral tablet, One tablet daily for two weeks ) [/i]?
Ideally I want this patient encounter to be recorded in ?Past Encounters? as a ?Telephone Conversation? I want to record the discussion between my staff and myself, and record the new medication ? nothing more (no vitals or PMH -just ?the facts of the conversaton and the med?
How are people doing this?

Use the message feature provided. Staff sends you a message saying blah blah called and they want blah blah, you answer, click on "Refill", write and send your script then send your note back to the staff who then calls the patient to tell them what you said and what you prescribed. Note is then saved to chart as a "Message"

"It's a good thing for a doctor to have prematurely grey hair and itching piles. It makes him appear to know more than he does and gives him an expression of concern which the patient interprets as being on his behalf. "

Agree with above. Messages are saved in past encounters same as addendums. Addendums allow you to bill but not prescribe. Notes the opposite.
Staff to Dr. Morgenstern: ?Message: Pt had lab work from urgent care faxed over because she is concerned that B-12 and, and pt complains of leg numbness. Would you be able to prescribe B-12 shots? Pharm: xxx-xxx-xxxx. pt xxx-xxx-xxxx . BW"
After you open this note you write (usually on the top portion over their message: "Her B12 level is only border line low, I?ve spoken to patient should do well with oral medication - I?ve called in : Vitamin B12 1000 mcg oral tablet, One tablet daily for two weeks"
You then hit refill and d/c pills, Rx shot to be picked up (or such) and then hit save to chart. Or you forward to staff to call her with additional instructions to save note to chart once that is completed.

The patient's expectation is that you have all the answers, sometimes they just don't like the answer you have for them
