We are in the process of writing templates and securing in our document library everything we need to assure we incorporate all of the elements of Bright futures in our Health Maintenance exams. Up to now we have been doing it in a less than formal manner but it appears to be perfect for Pediatrics to formalize this model in AC, I would be happy to share what we do as it happens but need to know precisely how to get the information from here to there in a usable format. It will include templates and documents linked to specific ages. I assume there will have to be two modes of transmittal, one in template format and the other perhaps FTP or linked.

On this topic, I spoke to a nice gentleman from California today about AC. He and two others are forming a practice or are in a practice. They are a pediatric group and are looking closely at AC. He is REALLY big on Bright Futures, wanting to know if AC offered it.
I steered him toward all the pediatricians on here, but I told him about your user name and how you would be one to talk to. Not sure if you want me to give your email to him. I think setting up Bright Futures in some turn key fashion would make the difference.
If you need any help with the templates, etc., let me know, but I am sure John can handle it.

The only question is how much of Bright Futures is copywrited. I know you can borrow somewhat and if you change the language it becomes new.
Since we have purchased Bright Futures is this sufficient?
I use a similar developmental guidelines I borrowed from the State of Illinois Healthy Kids Manual which I believe to be public domain. Pure cut and paste.
I would be interested in helping, sharing or whatever. It is for the greater good.

I think that is a great idea. The need for Age based Templates for the Developmental Exam and ROS is needed for all pediatricians. I made a few templates covering aspects of the DDST II exam that I incorporate in the ROS to cover questions answered on the parents PDQ. Lately Medicaid is asking for proof of a Developmental assesment tool, and the "PDQ" Parents Developmental Questionaire" is a good basic one, but not as complete as the proprietary Autism Screening tools or Ages and Stages, The PDQ is also much shorter and less time consuming than these later tools.

Bert, sry for the late reply. Yes you may give my email address to the doc in Ca. Re: developmental screening, the Ages and Stages seems to be the gold stanndard. I have templates for development for every age that I have written myself but as I turn more over to the nurses one of the things i will turn over is screening wih the ages and stages. In addition, I wiull continue using the MCHAT

You may be right. Oncloser inspection there is a copywrite sticker on the bottom of the page on several pages. However, borrowing from them is probably legitimate, espeically as I have in the past purchased a copy.
I had not looked at the Bright futures website in some time, but it had an interesting statement that said:
"To incorporate forms into an Electronic Medical Record System or to make multiple copies of specific items, please purchase a complete Tool and Resource Kit online."
However they also have all of the relevent forms online. Since primarily you are recereating the development, questionaire and guidance sections, all of that is available online.
All it takes is cut and paste to create a template.
I may have to look at updating my templates, something I have not done in a long time.

On September 23 all elements of the bright futures suggested health maintenance schedule will have to be individually covered without copay or deductible for any new policy written on or after September 23, 2010. It seems that any prior policies are grandfathered and do not have to comply.

There are multiple sites for Bright futures information and forms. I, too have purchased the kit but I think many of the sites allow reuse. I did not see the copyright sticker on the pages I reviewed. Could they be for forms created elsewhere and incorporated into bright futures? Some of the questionnaires seem to have that and a requirement that it be purchased or use be licensed.

CAN ANYONE PROVIDE ME WITH TEMPLATES? I RUN A BUSY URGENT CARE CENTER AND COULD NOT FINISH THE CHARTING ON TIME DUE TO POOR INCOMPLETE TEMPLATES! TXS HURAIBI

I designed templates for the different ages for the scheduled well checks. I put these into the hpi. The problem was that there were SO MANY items to cover, that to do a credible job of addressing each item and acurately documenting each item would take a really long time. Not only too long for me to be efficient but the parents would start getting impatient. So, what I do is- First, Parents are given informational sheets including the bright futures topics and concerns that are developed for the different ages for the scheduled well checks.. Families are supposed to read these and ask questions and discuss concerns. I am thinking about having audio files to help the families that are not strong readers.
Then I have questionaires that have the bright futures topics that parents can answer by circling. The ma helps those who cannot read well.
When I go into the room, I review the questionaire, address my concerns and the parent concerns.
I then have age related templates in the hpi that refer to the information sheet and the topics covered , and also to the above questionaire that has been scanned into the chart. The template is very focused and allows comments for areas of concerns for me and for the parents. This has been very efficient so far. If I need details about specific items covered, i look it up in my scanned items

I too, designed my own forms a long time ago, and they have served me well. I also have accompanying liturature which works for me. I beat Healthy Futures to the punch many years ago, but... a requirement for developmental evaluation tools is that they have been validated. Thus Healthy Futures.
In all honesty, I could have probably just done a few for developmental screening, but I have done all of the developmental forms.
I have always put my developmental in social history and continue to do so. I know Jon put some development things in the ROS, that doesn't seem right to me but everyone is different.
I have uploaded these templates to the AC site.
It is an involved process.
Step one was extracting the data Step two was formatting the data. I indented (2 spaces) the items under physical development, cognitive development, ect so that a NO could easily be placed indicating abnormal. Step three was to look at the wording so that it was defaulted to yes or normal Step four was to compare with my old developmental sheets and mix and match.
They are uploaded with the statement Based on But not Quite Bright Futures.
I still have to do the plan section. Not as worried about it. My old templates work OK. To be consistent with the handouts, it should probably be updated. I might get around to it at sometime. Like all of you, I too am busy seeing patients.
Huraibi, Please do not use all caps. It is the equivalent of shouting. You can always make your own templates by right clicking and hiting the ALT button at the same time and edit and save the text you would like. See Richard's note above. It takes time to learn how to use an EMR. You need to delegate and use checklists ahead and it will speed up the process.
It's fine to use other's templates, sometimes you just have to make your own.
