I discussed our new practice narcotic policy with %FNAME, including the requirement of follow up office visits for all future refills for controlled substances, random drug testing for both the prescribed medications and other prescription and illegal drugs, and random pill counts for controlled substances, with immediate dismissal from our practice for violations, and %he understands and agrees to comply with these policies.

We discuss with each patient who receives either chronic CIII or ANY CII meds our policy, and randomly test. (see other template for testing)

Of course, that was from when the policy was NEW.. sorry, edit out the "new". Has been the policy now for almost a year.
