We were early adopters of Updox, and have generally been big proponents. I view it as two connected programs (and in fact you now could say it is three). The first is for document management, the second as a patient portal, and the third is a telemed platform.
For years we only used Updox for document management. Sending and receiving faxes, importing into AC, etc. All things that can be done other ways, but simplified (in some cases, significantly) by Updox. I used to say that if I got rid of Updox my office staff would quit because it made their job so much easier.
On the other hand, as noted here, this can be done other ways. Leslie was an early advocate of Paperport and while she has not posted here for years (or used AC), Donna is one of her acolytes and still successfully uses that program. I will touch base with her and see if she is willing to stop by here in case people have questions.
The document management portion of Updox has changed a bit (a desktop version was eliminated) but it generally functions much the same as it did 10 years ago.
The patient portal is mandated by MIPS, etc. I think AC itself comes with a form of it, but the separately purchased Updox by subscription has a portal that is much more robust and well integrated. Both have their limits. We use the portal more now than in the past, especially with the advent of telemedicine.
Finally, for an added price, Updox has a telemed feature. Better integrated with AC but though we pay for it, I am not sure it is worth the cost vs free products like Doximity dialer and doxyme.
In summary, features have not changed a whole lot since the early days. We don't need support often, but have not had major issues reaching them when it is needed.
As to cost... that is where things have changed over the years. Due to changes in the way they calculate costs per number of faxes, and simple price increases (as well as the telemed changes) costs have risen dramatically. We were paying $30 a month in 2012. Now it is like $180 (though with two providers instead of one, telemed, and lots more faxing). I need to take a hard look at whether it is worth it or not.
