Hi everyone,

At least part of the above discussion seems to refer to messages becoming "detached" from the patient chart. We have been experiencing this, very intermittently, since approximately version 4, perhaps version 3. Just to show that there is nothing new under the sun, this link from 2011

talks about detached messages.

I don't think we've ever had a message attached to the wrong patient, but when we have several patients with the same last name and first initial, we must be sure we reattach it to the proper patient.
