You got me looking closely at the CCD. While I don't see discrepancies in medication lists, I have just discovered that the CCD doesn't include any of the "Plan" in the encounter. Only data put in the assessment box is included in the encounter. Anything in the Plan box is excluded. That's a huge problem.
