In both cases the patient/client could care less what is wrong. They just want it fixed.

Your patient is going to scream about that surgery and try to get it for a lower cost rather than live in pain. My client is going to demand that I implement that feature the way they want and to hell with everyone else.

We may be using different oars, but we are in the same boat.
