You can do it in 2 ways
1. Make a template in WORD of the ACOG form and store it outside AC on a shared drive.When the patient has delivered the template can be converted to a PDF file and imported to the pateint's chart in AC
2. Use the ACOG form and use a free PDF editor for each visit and IMPORT it to the patient chart in AC.
