I keep Diabetes flowsheets as Excel files stored under "imported items."
We keep a master template in the AC folder called "Templates."
When I have a new diabetic patient, I import the diabetes flowsheet from the templates folder. Customize the form for the patient. Done. Each time we go in and add the new A1C value, document a sensory exam, ask if diabetes eyecare is being done, add the dates of the prior microalbumin/LDL/Cr.
One could easily do this for the ACOG form.
