Could you tell us what your trigger was? I have a similar thing but I haven't seen a definite pattern.
It's quite frustrating.
Were you still on 3.7 in January? I had a similar problem with 3.7, I found the trigger, but I couldn't say exactly why it happened.
I don't believe it effects 4.0.60
