I know what you mean, as a Neurologist, I just attach my note to the cover page when I send out follow up letters and the empty spaces don't look that great but as far as I am aware of, there is no way to change that. The alternative, if you want to do the extra work, is moving the particular pieces of data (such as HPI, meds, etc.) in the letter writer, to the letter you are sending out--unfortunately, this takes way too much time, in my opinion, so I quit doing that.
