Hello
We have upgraded back in May to AC v12 build 329 . We are now seeing missing drug history for a number of our patients. AC tech support is saying this missing drug history issue got fixed in the newer build v 369 or so.
Can anyone please comment they have experienced the issue and saw it go away with newer build such as 369 or so ?

Do you mean the medication list or the drug history you can lookup in the prescription screen?

I'm on 369 just prescribed tramadol for a patient it shows up in e-prescribing transaction log but not in patients Current Medications nor in the Plan

I meant drug history that shows med filled in the last 4 months. It shows drug prescribed by any provider including nursing home.
This is the 5th item on the Medication and eprescribing toolbar.

I can get the list of meds prescribed in the last 4 months from about 40 % of patients. I am on V12, 369 and this was the same in the previous version, so no change after upgrade. Just checked to see if meds prescribed yesterday are in the drug history and none are present in the list that comes up. Wonering how long it takes for them to show up in the drug history after they are prescribed?

I usually can get information here but sometimes have to extend the time and refresh. I do not think this is an AC or Newcrop issue. I think this is an industry wide issue and is based off of the insurance claims for medications. I have found that my cash paying patient's medications usually do not show up.

I have accepted the fact that most of the time no results will be obtained and do not depend on the drug history. If the patient went to the ER or urgent care and was prescribed a med I will try to do the drug history to find the med, A lot of times without success.

Raj, next time change the date and refresh and see if it comes up. I have had that work for me.

We were prompted by AC to upgrade to V12 build 369 as this was thought to have cured the issue. We were in V12 build 329.
We still see the same issues after upgrade to AC V 12 build 369
Drug history is critical for Medicare patients Med Adherence measure. That is the only way to see if a medication is filled in a timely manner and if not filled then take actions.
I contend the drug history needs to be there to reduce unnecessary overhead calling the pharmacy to verify. Stain, HTN and diabetic Med Adh are all triple weighted for HEDIS Star score. Drug history allows us to keep a patient compliant.
Also for many of these patients, it has been working for us up until Aug 30, 2024. So this is a new issue.
And yes drug history shows only when filled and paid by insurance. Cash transactions, Good RX transactions will not show up

I just had a patient today where I tried the 4 month Drug History did not return any results. I changed the months to 12 and everything showed up including medications picked up last month.
Not sure why the 4 months is not bringing up the list but changing the date range has helped on several of my inquiries.

Over the years I found drug history to be complete about 95% of the time. This is the first time I've heard of getting results by changing the lookback from 4 to 12 months.

Found one today that I can not get a med history.....even after changing the date. I also found a problem in the med list that was caused by a refill request error.
