When a new note is started by my staff at patient check-in, the Past Medical History and Family History are erased and not carried over to the new note. Social history remains intact, however.

I can recover these from a prior note, but that's extra steps having to drop down the most recent note, and then erase the HPI, exam, etc. from the prior note.

Is there a setting I'm missing? Has anybody else experienced this issue?

Is the staff member choosing to start a fresh note as opposed to starting from the previous note - which would have all of those things carry over?

It sounds like it could be as simple as how the staff member starts the new note.

Thanks for the reply Indy - they are starting from a previous note, putting in a new CC and clearing the HPI field. I can see past medical and family histories in the prior note, but then when the note is forwarded to me after check-in, they disappear.

This didn't sound familiar to support, may have to demonstrate to them remotely. I was just wondering if anybody else had encountered this behavior, and if there was an easy fix.
