We're just starting to receive Transition of Care documents. Mostly it's notices that a pharmacy has given a patient an immunization. Has anyone figured out how to use this feature effectively? It seems poorly designed. The info in "Help" is not very enlightening. It asks if we want to reconcile--but it doesn't seem able to reconcile the fact that the patient got an immunization. You have to do about 20 clicks just to find out a simple bit of information. Anyone out there had a better experience with Transition of Care??

When trying to open the message from updox in the Drs. AC inbox a window opens that says "The requested document is not available from the Amazing Charts Central Server". Anyone know how to fix this?

Is there anyone that can help me figure out how to open transition of care documents in inbox? Ours won't open and we have 0 for numerator for request/accept a summary of care.

I have tried to use it also, never seems to be open with any info. About the only thing that works, when you close the greyed out data, it asks if you want to delete the message. Deletes really good.

Finally got transfer of care documents in inbox to open and able to move to charts, tech support had to log into server & fix. However, it seems we are getting the same documents in inbox up to 4 times. This is rather irritating though if you get 4 or 5 referrals and 4 copies of each one it fills up the inbox real quick. Anyone else have this happening?

I may be a bit behind here on AC versions. What AC version are you running?

Did AC already release a version that allows to accept transition of care from specialist ?

We have Version 10.0.2

I do not think 10.0.2 allows you to import transition of care same way we import lab today.

AC is working on a newer version to enable that feature. That was my takeaway when AC was doing a conf here in Houston back on Sep 2018.

Stay tuned for the arrival of the next version that should enable this feature.
