We are an office where taking vitals is not necessary and although it would be nice to get that off the charting template (can't figure it out), there is actually some wording next to "Vitals" that shows up only when printing the note and I need to figure out how to remove it. Anyone have a clue about where it might originate from? I've looked in the Admin Options and the template area but it isn't showing there so I'm really confused. To be specific, this is what shows up when we print a note:
Vitals: Pain: 0. I need to have "Pain: 0" removed as it is not relevant for us.

I am not sure what you mean by "template". Are you right clicking in the PE area and selecting a specific template for your notes?. I have tried to see if this is occurring in my notes. We most of the time for sick visits only do the weight and temperature. All our notes at the beginning of the notes ONLY have weight and temp at the top when printed. The only time you would get a "pain:0" would be if you click the "note" button under the vitals ,select "pain scale" and place a number 0-10 in the field. Usually there is no number in the pain scale field. This , of course should only be done at the time of writing the note. I would suggest to make sure no number is populated in the pain scale field before completing the note.

I thought it was part of the template but it is actually part of the program. We have to manually put in a zero because our only options are entering a 0-10, it won't let us leave it blank. We don't want it to show up at all because we don't evaluate pain levels on patients and when we have a personal injury patient, this "pain" field is causing us pains!

What version of AC are you using?
We are on 10.02 but I think it is has been like this for some time: the only thing that shows up are categories of vitals that have been entered. In fact, the word "vitals" does not show on printed notes.

The first line of our exam typically looks like this: "Physical Examination: Wt: 222 lb Ht/Ln: 64 in BMI: 38.1 BP: 156/93 Pulse: 79"

If we enter only a weight, it is "Physical Examination: weight: 222 lb"

If no vitals are entered, after "Physical Examination" and what next appears is the text in the exam box.

All of this means that you should be able to achieve what you want. Unfortunately I am not 100% sure how to do it.
Next to the box where you enter the weight on the visit screen, there is a box labelled "Note". Click that and a box with 4 tabs opens. MAYBE something is clicked in the pain scale box (a zero, perhaps) and MAYBE the program remembers that since you last recorded a pain scale, it will remember that on all subsequent visits. So... I would try opening each of those tabs and make sure nothing is checked, then save and close, and to be safe, exit AC. Upon restarting, maybe the pain scores will go away.

Try that and let us know.
