How do other Pediatric practices manage when you have newborn and child notes where the smoking history is a default template. It seems to appear every time you open the note if not completed. Do you just manually delete each time?

I am not sure which version you have but I am on version 9.1.0. When a chart is opened the "smoking" part starts of with "select tobacco use" and if nothing is chosen, nothing appears in the chart. I wonder if this has changed in future versions. Raj

Thank you for your reply. We have version 9.2.0. Under the social history where it states "select tobacco use" I usually choose nothing and find in the social history: Tobacco: {Pipe Smoker: No Cigar Smoker: No Chewing Tobacco user: No } This is not appropriate for a social history of a newborn. I wonder if the template is a default based on age, e.g. automatic on all teenagers?

Apparently this has changed since the version we are using. Agree that this is not appropriate at all. More clicks to make a note more appropiate is not what we call progress. I did not upgrade to your version so I cannot comment if the template has a specific default.

Nunez, On the social history field, click the green button to the left of the field to open up the expanded smoking. There are dropdowns for "Answer" and the selection is "yes" or "No". Either selection puts the template in the field. Make sure the answer is set to the blank line above the "yes" in the dropdown. This will set it as "no answer" and will not add the template to the field. Here is a pic of what I am talking about: ![[Linked Image from amazingcharts.com]](http://amazingcharts.com/ub/attachments/usergals/2017/10/full-3770-988-smoking.jpg) If you have issues, just send me a message.

Why doesn't the smoking history automatically update? I entered "30 years since last smoked" last year, and it still says 30 years a year later. Seems like something an "Electronic" Medical Record should be able to do.
