We provide clinical summaries to our patients for each visit.... does this suffice as "Provide Summary of Care records for each transition of care or referral????

Hi MGulledge,
No, providing the patient with a clinical summary does not suffice for "Transition of Care". You need to send a "Transfer of Care" document through secure messaging to the provider you are transferring the patient to, or referring the patient to. This must be done for 10% of the patients you transfer or refer.
