How is it that I just found out about these?
What are replacement encounters and how do I use them to submit these cpt ll codes?
Yes Doctor there are cpt lll codes (please don't go there yet).

As Vince Lombardi once said "What the hell is going on around here?".

Scan Health (California dreaming) plans is the only info I can find on this .

The way I understand them is that they are simply tracking codes for those involved with managed care, IPAs etc.

We had to learn on our own how to bill for them with some help from the IPA. Most of them are billed with a 0 dollar amount since they aren't for payment. Our PM didn't like 0 so we had to go with .01

Keep in mind though that your specific ACO will tell you specifically which codes they want reported. Seems strange that they are just now telling you.
