Our practice is on 8.2.4 and we have an interface with Kareo so our encounters crossover to our PM. I would like to know if anyone else is having issues with Phantom vaccines showing up on encounter then crossing over to their PM that were never given? I even have vaccines showing up that are not in my order set anymore. This makes life very time difficult as I have to go back and check each encounter to make sure it is correct. AC will also randomly picks DX codes from the active problem list not from the encounter problem list. I would like to know how other office are handling this issue. AC just keeps telling me that this is a known issue. I would think that for all of the customers that have interfaces this is something that they would want to fix? Way would I want to keep my interface if it isn't working correctly?

This had been an issue for us. It happened with version 7 and 8.0. I usually finish my notes at night time and after I complete my notes I would finalize the charts. All vaccines that were ordered that day would have their orders completed by the nurse and the vaccines given that day would appear in the final window to select type of visit and procedure codes. There were times however when I would finish the note BEFORE the nurse would complete the order(By selecting the order, vaccine given and lot number). When I finalize the chart then I would notice that the vaccine codes were not in the final window and I had to manually put them in. If the patient returns for a another visit when vaccines were not given, when I finalize that visit the vaccines given at the previous visit now appear in the final window and I have to delete them.
So apparently the nurse finalized the vaccine order after I finished and finalized the chart.
I can see how these vaccines from a previous visit may become phantom vaccines at the next visit and be imported by the PM.

After reading the long-winded explanation above, it may seem that these problems would not occur if the doctor finalizes the note AFTER the nurse finalizes the vaccine order. Just my suggestion. I only upgraded to version 8.2 last weekend and have not seen it occur yet.

I have never reported this problem to AC because I just make sure that I finalize after the nurse finalizes the vaccine order. If I finalize a note and see these vaccine codes I know that I closed the chart before the nurse finalized the order at the last visit and I delete them. I do not have a PM that imports the data from AC.
