This is probably best served as a procedural question. When my nurses send a Chart to get Signed Off after a patients' Encounter, in a perfect world you could sit down and review - line by line - what the nurse keyed in for the SOAP Note. Well, clinical life just moves faster than that and invariably some SOAPs get Signed Off with typos and values that are transposed or simply out of place. Basically we're talking about clerical and data entry type errors.
This is particularly rampant in the free text sections of the Templates. I have noticed that in other software, you are provided with some sort of Preview window that displays a full summary of all the data contained in your Note; this Preview function gives you the final opportunity to go over your Note and double-check for any final errors and fix them BEFORE you Sign Off the SOAP Note itself. Once you Sign Off a Note in AC, that's it; this mimics real world medical practice protocol. I realize and understand that the AC Addenda is the recommended tool for these instances, but I feel it's still inefficient to use an Addendum just to go in and fix something like a typo.
How do others deal with this procedure or process? Surely I cannot be the only one that has these problems. What do others do in order to ensure that your NOTE is completely "clean" so it can be Signed Off without worry? At the end of the day, all we want is clean, error-free/typo-free SOAP Notes. But not having any functional way to do a final check prior to Sign Off is something that constantly hampers my workflow. How do you deal with it? Any suggestions?
