Is there any way to change the headings of the sections? I would like a heading called "Problem list" or 'Diagnoses". CUrrently I am putting this list under past medical history. Patti

No, but of course there is an actual Problem List under the Summary Sheet tab.

As a matter of fact, I do something similar to what you are doing under Past Medical History. I have a PMH template for:
  • PAST MEDICAL HISTORY:
  • PAST SURGICAL HISTORY:
  • OB/GYN HISTORY:
This looks funny when you print a normal clinic note, as the Past Medical History starts with:

Past Medical History: PAST MEDICAL HISTORY:

But I only *print* my notes in the LARGE format and it looks just fine.

I have often asked for a window which is quite different from the summary sheet. I don't think we will ever get one, because I think the author of the program basically considers the summary sheet just that.

I would like an option which allows one to have the program open in Face Sheet mode (for lack of a better term) which would show a list visit diagnoses, problem list, medication list (both active and inactive and why), allergies, etc. I would like it to be editable so there could be a section for consults seen or hospitalizations and dates. Being able to switch back and forth would be nice. I would also like to see something that isn't done on a paperchart nor on here, but some way for the plan from the last note to show up for the new, so it would keep you from having to check back to the previous note.

Bert's idea would be simple fix for the programmers, because they already do this for the PMHX, FMHX, SocHX. These are carried over from one note the next to the next, etc.

Simply make the Plan section the same format, then edit this section as you see fit....leave it the same, change it around, keeps parts and delete others....whatever you want.

This would be awesome, because in FP we do a lot of the SAME STUFF, OVER and OVER.

Plus, if nothing else, this allows you to see what you did last time, delete that and start anew.

Exactly. The two biggest areas where I end up putting my foot in my mouth (OK in AC and pediatrics anyway) is recommending medications which the patient has already been on and recommending a plan that seems inconsistent with my last note.

You fellas just lost me: it's already there, isn't it? When I open a new note, all the data from the last note is there except the vitals.

We're talking about adding the same feature of PMFSHx to the PLAN section.

In this manner, the PLAN from the last visit is brought forward, just like the other sections are brought forward. Then you could at least review your plan from prior visit. You could then choose to keep it the same or modify it. At least you would not have to look back to the old notes (clicking the tab) in order to see what you did.

Okay, I am still not sure I understand what you are after. When I open a new note, all the data from the past note is carried forward to the new note, and everything has to be deleted that does not pertain to the new note.

This is either carried out:
  • manually, field by field
  • automatically, by File->New Note->Blank Note (clear all fields)
  • automatically by File->New Note->New Note (save PFSH), or Ctrl-N.
The only "feature" that the PMFSHx fields possess, if you want to call it that, is that they are not deleted when you hit Ctrl-N.

Are you saying that you don't want the Plan to be cleared when you hit Ctrl-N?

In my mind, if you want the Plan carried forward, either clear the unnecessary fields by hand, or else have your assistant hit Ctrl-N, and then have them copy and paste the Plan from the last visit.

This is EXACTLY what I want. That would save staff (or me) from doing any copy/pasting maneuvers that are just one more click in a long sequence of clicks all day long.

I wish this could happen.

In the meantime, I would

  • Open the chart.
  • Highlight the Plan section.
  • Ctrl-C.
  • Ctrl-N.
  • Click your cursor in the Plan section.
  • Ctrl-V.
That's the fewest number of clicks I could do it in. HTH! grin

I like this tip. I frequently use the PLAN as a "to do" list for upcoming appointments. Such as lipids due after March 17th, 2008 or Colonoscopy due 2009, DEXA to be repeated August 2008, etc...
