By default when opening a patient's chart,the "No New Diagnosis" button on the Problem List section is checked. How do I reset this so it is unchecked when opening a patient's chart? I have had a no. of patient charts with the "#NO CURRENT ACTIVE DIAGNOSIS" below 10 different diagnoses under Assessment.

Just to the right of the diagnosis field, there is a check box you can use to select No active diagnosis or show last diagnosis. For me, it stays where I last left it. So, if unchecked, the next time I open the chart, it is unchecked. Is yours different? I think he is asking (as I have wondered) can the default setting be "unchecked", rather than having to do it each time the chart is opened?

OK.
1. Checkbox, signout and checked box is there again next time 2. Change to not checked, sign out or close and there again next time 3. Check or uncheck, close AC, open chart it is still the same 4. Check box sign out, open chart, still checked. 5. And the grand finale, Check box, go to a different computer, open chart it is checked. 6. Uncheck box, close chart, go to another AC on a different PC, open and it is now unchecked.
So, every way on try on 6.1.2, it stays the same.

You're right. When I upgraded to 6.1.2, the checked No New Diagnosis checkbox appeared. You're right, once unchecked, it remains unchecked however, I would still prefer that this box is by default, unchecked. Thanks again

IT doesn't work. it is random. i don't know why on some charts it says no active diagnosis...even when i am seeing someon for a f/u 3 weeks later sometimes that box is checked...even when two other diag are still in the active window...don't understand it.

I don't think it is saying there are no diagnoses in the window above it. It is not saying that the patient is a new patient, so there are no diagnoses IF the patient is indeed a new patient. There would be times that a patient comes back for a follow-up. Is having sinusitis there a good thing? I don't think so. Why would anyone have a diagnosis already in the note before they have seen them. From my point of view, I want to get to the Exam and/or labs, come up with a diagnosis and add it. I don't think there is a high probability that the diagnosis I come up with will be the same one that is already there. No, there is no "active diagnosis." Because there is no diagnosis. I don't even know why it is there. Maybe we are talking about two different things.
I always find that when you compare an EMR with the thing it replaced: the paper chart, they should be rather similar. I never saw a patient, wrote a progress note, and already had a diagnosis in place.

I thought this was for taking care of healthy people, as in no chronic disease, no active problems, no active diagnoses. Not my situation, as no primary care. All of my established patients have an active diagnosis.
Anyway, I only worry about what I'm going to put in the Assessment box at each visit. And that is almost always chosen from the problem list, because chronic care is supposed to be easier than primary care.
