Is there a generic form that can be used for a patient to file an insurance claim with any (well, most) insurance companies? Similar to the HFCA 1500 Form that is kind of a generic form for the physician office to file claims.

The idea for me is to be able to tell a patient "Well, we do not accept your insurance, so you will need to pay in full today. But we can file the claim for you." Or at least quickly fill out the claim.

This is a great idea actually...It would even be good for patients paying out of pocket but need to show it to someone (HSA, etc).

I may misunderstand your question, but you can easily get a superbill for any patient. In patient list either right click and select pull superbill or open chart and select last tab (account information) there is a button for hcfa 1500 among the choices.

Yes, I can get a superbill. But I cannot submit it to my clearinghouse (well, I don't Think so.). The HFCA form is for us to submit our bills.

I literally want a general form that can be used to file claims for self-pay patients for the patients to be reimbursed. Because you don't take their insurance, and this lets you add a service in to make things a little easier on them. You can even charge an extra nuisance fee (For just $15, we'll happily file your claim! Have a nice day! )

I think you could give them the HFCA (CMS-1500) form with box 13 left blank and box 27 checked "NO". Or you could mail it in for them to get that fee.

Box 13 assigns benefits to the provider, and normally ours says "Signature on File". Box 27 accepts assignment which would be checked NO since you are not a participating provider.
