I found this tip for resurrecting a lost note posted by rdoc in may 2012 and I'm reposting it because of how much it helped when i learned today that I lost a note in November.
The patient called for a med RF. Her AC chart showed no visits. Her paper chart showed visits for previous years only. We had a paper fee slip and insurance billing for the visit. And we noted the AC chart did have, under RX, the Rx that I wrote in November. So I probably had closed the chart without saving it.
Using rdoc's tip, I went to the Inbox tab at top, changed it to Deleted Items. Took AC a few secs to comply and populate the list. Then I could scroll by date to the message with the pt's chart, retrieve it to my Inbox and finish it. That way all the vital, hpi, ros, fam hx, etc that my MA had entered were resurrected. I still had to reconstruct my part of the note but that was much better than not having the vitals, etc.

you can set in preferences when you want deleted items removed. If you leave them indefinitely they will be there for you when you need to go back as you did above. However, they can start to bog down the system, it seems, if there become too many. I have mine set to be removed when I close AC.
