In the ED reports from our hospital under ROS, they always write the same thing:
A complete review of systems was performed. These are otherwise negative or normal.
Is this good enough for coding or good medical documentation? What is a complete review of systems?

You have to mention positives or pertinent negatives in either a problem focused fashion, or else 9 to twelve categories, depending on the level being billed. The statement above will not pass muster with the Medicare bounty hunters.
