I was under the impression that to meet this all you needed to do was mark "Medication Reconcilitaion" in the medication area. This has been done by my nursing staff but it is not counting. Is this menu actually only meant for new patients into our practice? If that is the case then do we need to just check the "transfer of care" box to count?

I always double click the medication list - review any changes, add new OTC other scrips from other docs, inactivate meds that are done, etc - then hit MEDICATION RECONCILIATION AND save. It always counts for me - are your nurses by chance doing this under their name and then putting chart in e-mail to you ? I remember that this seemed to negate the reconciliation. If so you need to do again.

Yes, my nurses are going over the meds with the patients. They then forward the chart to me. I believe when we first started it was counting towards it but now it is not. I will try doing this myself and run a report to see if this is the problem. Thank you

I did this myself and it does not count towards Menu 7. It only counts if I click the transfer of care box first.
