I am new to AC and would love to know if it is possible to create a template with drop down options or bulletted, so the practitioner fills puts a ck in the box or selects an option from a drop-down list. Has anyone had any experience with this?

There is no way that I'm aware of to add drop down options. The best way is to make separate templates for each instead of using drop down boxes (or bullets).

What is it you are exactly trying to do Karen? There may be alternatives besides AC to do this?
