I am a new user of AC going through the trials of a newbie. I have inadvertently added some diagnoses to the problem lists when trying to enter the diagnoses when creating a new chart and do not know how to delete them. Some very embarassing diagnoses have inadvertently been entered with a click of a button. I sure do not want these left on a patient's chart! Any help would be appreciated.

open the patient's chart, click the "summary" tab (second tab from the left). you will see "problems" panel in the upper left of the summary tab. From here it's pretty intuitive. You can add, remove, edit problems.

It happens to us all.
Just go to the summary sheet, and there you will find an option to remove. (A quick way is to hit F6 after you highlight the patient's name in the main list)

Thank you all for the quick reply. Does anyone have any recommendations on where to go to review basic procedures such as this? Obviously, I was clueless.

Is there a way to delete an encounter once it is signed off on? I got in one Sunday and updated a medicine and all the fields were filled out from a previous note (3 days earlier) and i signed it - but would like to remove it. Maybe it doesn't matter much since there is no billing involved but I suppose it will get counted in the meaningful use criteria.

I recommend you just sit and play around with it for a while. Go to every screen and try all the buttons to see what they do. This always seems more helpful to me than a printed write-up. Of course there are plenty of answers on the board here.

you cannot edit an encounter once it is signed. You can create an addendum, but cannot delete the note. It's part of the legal security required of an EMR. In the event of a lawsuit, the lawyers don't want us deleting important office encounters. So we are locked out from removing office notes. Every note would get counted toward MU.

I agree with Boondoc. Play around with it. Try to break it. click everything. RIGHT CLICK everything, there are tons of hidden features that involve the right mouse button.
