Salem, when you sign your note it will be saved. There is also a tab called Past Encounters where you can view any previous encounter.

Maybe I am reading too much into your post. Are you trying to transfer data into AC from the old Problem list / medical record after hours? I had a template that I used at that time for the Chief Complaint: "To Merge Records" (so that when you review "Past Encounters" you can skip this visit, you know it was just for booking), and then under HPI: "This entry does not reflect a patient visit, it was used to update data under Past Medical History, Family History, Social History while reviewing the old, paper chart." That was for myself when I can't remember what I this was 10 years from now, but also for any others, who might be trying to review the chart.
Now you treat the entry like a visit, enter anything you want, including the diagnosis codes that this patient will be needing. That saves a ton of time later. Then sign the chart, with no charges. It will ask, do you want to add charges, you answer "No" and it will save just like a visit. This works, and it has the advantage of keeping a "paper trail?" of what was charted, and when.

We are finding it helpful to do what Martin suggests in the evening or weekend for the coming week's scheduled visits. It relieves a lot of stress at the time of the visit.

Have faith. You are thinking it is going to be a long year but in reality you start to see the first of these follow ups in as little as a month, and the payback in time, Rx's, Dx's etc. is Awesome. Makes the rest of year fly by! (Lying like a rug, but it does seem better the more second and third visits you see!)

i appreciate the input from every one . I did not expect a quick response. I learned few tricks. Sepcially the one about merging records. My question was about simply trying to inport some data from previous encouter in amazing charts or previous vist. Occasionally, I open a patient chart I found no PMH Or Family history. Is this possible ?

You have an option with respect to how to begin the note. There is a drop down box that lets you select a completely blank note, a prior note or just to save the allergies, PMHx, Fam and Soc Hx. Maybe you have a staff (or yourself on a different workstation) with the wrong option selected. (Don't forget, just click Ctrl-N for a new note).
