Does anyone have an easy system for requesting medical records? This is increasingly taking up my staff's time. The time they spend on hold requesting labs, radiology results, etc. is quite frustrating, especially when the place they are calling says they will fax over the information but then never does and they have to call back.

So I would like to create a standard form that contains what information I am requesting, and a signature from the patient / POA authorizing release. I think if I fax a request instead of have my nurses call, it might save time. Kind of like how pharmacies fax refill requests to doctor offices instead of calling us.

Does anyone use such a form to be able to give me an example of what it should look like?

Lee, if you go to our website you can print our form and use it as a model to make your own. FYI, this form is based on a form provided by our malpractice insurer as a suggested form to use for this. www.doctorcoghill.com Then go to the page named "Forms & Privacy Policy". I usually send the form with a brief cover letter.
