I was wondering what others were doing to document calls attempted.. lets say when delivering lab results and such. We try to reach a patient three times. We used to document each attempt and date on the actual paperwork, but we are letting go of the tricycle wheels and relying on pure AC.

I send the e-mail to a staff member on this to have them call - you could have them hit reply and send to themself - type in that they attempted to call and got no answer, left message, etc. Do this again the 2nd time and on the 3rd time send it to you so you know. You could then either file it or generate a quick mail letter.
Just an option - then you save to chart and paper trail is intact. If I have a phone call I am returning I will just hit reply - type what I want for information after talking with pt and save to chart.

Ditto to what Steven said. We also have patients fill out a "how may we contact you" form at check in. On the form it asks where/how patients want to receive lab results and if it is ok to leave a message on voicemail or with spouse(this has cut down a lot on repeat phone calls to try to deliver test results, esp to folks who are at work the same hours that we are).
