Has anyone ever lost a SOAP note? A patient called today about signing some papers for his visit. He appears on the schedule. His visit was billed (and transferred to EZClaim), and the medication I ordered was entered on the med list. However, his SOAP note is missing. Is this common or even possible?

One thing I thought of was that AC crashed in the middle of signing a note. I subsequently traced it down to be working on the note while the database on the server was being automatically copied to the internet using KeepVault Backup. I suppose the copying process would lock the entire mdb file and thus made it inaccessible for record updating.

Has anyone ever lost a SOAP note?

Jack,

I have never lost a soap note. Yes, I have not been able to find one, but I don't think I signed it and it was in the database and then gone.

It depends what version you are using. V3 uses .mdb files and they are less stable than .mdf files. I understand your auto backup and it seems helpful, but I have found that it can cause issues with the overall running of your program. From my perspective, you would be better off either not using it or setting it for longer periods of time.

But, bottom line is it is most likely something that happened during the note process. I hope you find the note

Jack,
You must be using V3.7?
Please strongly consider upgrading to V4, it is MUCH more stable than the .mdb Access database system. SQL server accesses the database WAY faster than Access.

Yes, I should really upgrade to V4. However, I am waiting til I have some time and a new SBS2008 server. I am a little apprehensive to change when 3.7 "aint broke", but I guess it is in the process of breaking.
