Has anyone ever lost a SOAP note? A patient called today about signing some papers for his visit. He appears on the schedule. His visit was billed (and transferred to EZClaim), and the medication I ordered was entered on the med list. However, his SOAP note is missing. Is this common or even possible?
One thing I thought of was that AC crashed in the middle of signing a note. I subsequently traced it down to be working on the note while the database on the server was being automatically copied to the internet using KeepVault Backup. I suppose the copying process would lock the entire mdb file and thus made it inaccessible for record updating.
