I have never lost a soap note. Yes, I have not been able to find one, but I don't think I signed it and it was in the database and then gone.
It depends what version you are using. V3 uses .mdb files and they are less stable than .mdf files. I understand your auto backup and it seems helpful, but I have found that it can cause issues with the overall running of your program. From my perspective, you would be better off either not using it or setting it for longer periods of time.
But, bottom line is it is most likely something that happened during the note process. I hope you find the note
