I wanted to bring this up so hopefully you are not missing things too. Here is the issue....Importing Items Sign Off: Imported items no matter what format (pathology, consult, photo etc) are missing the comments I have written and the signature. According to one of VP's today this is not limited to my database as they have duplicated it on theirs, but have not heard from other AC users. It is not happening in the labcorp sign off page. Hope this helps! For now we are printing and signing off and importing them. This will not be resolved until another update.

That's pretty interesting, we've also had some trouble with labs (though I insist it was an MA misallocating incorrectly named labs). *What version are you on, and does this happen just for you or does it apply to other providers too?* I gave my NPs a list of items that needed to be looked at today but they mostly belonged to just one of them

Benjamin, I am not sure you should necessarily blame the MA.
I have occasionally been missing comments on signed off labs from the get go, ( Started using AC 4.0.60 about 3 months ago, and now am on 4.0.72). The problem ( pretty frustrating) is intermittant and therefore pretty hard to pin down. I have seen several different comments in different threads related to this.
It is a real safety problem, just today my MA asked me about a patient with moderate dysplasia whose path report was in the chart signed off but no attatched comment. ( I remember that I had sent a "sign-off and send" to the MA that the patient needed treatment when I saw the path report.) Sometimes my messages from the lab sign off goes to their deleted box instead of their in-box, I have taken my lap top over to their work station and watched this happen. The person whose messages are most likely to go to their deleted box is the person working on another lap-top that is not hardwired to the domain. My Tablet is the only computer that is still not on the domain ( for reasons that I won't bore everyone with) and is functioning from an older network setup that my office had been using for more than 10 years . I have not pressed my problems with "lost stuff" with AC support because I have been attributing it to network problems at my site.
So, I have noticed at least 2 problems: 1)sign off messages are "lost" because they do not go to the in-box of the recipient. 2) The sign off comment is not on the lab when you subsequently open it up in the imported files section. I have not stopped to see if this might be that comments attatched by "sign off and send" get lost as opposed to comments attatched to simple " sign-off".
In early July I had set up the lab interface with Quest to import labs directly trying to eliminate the step of having someone in my office scan in the lab report, but Iwas having so much trouble with lost messages attatched to the lab reports that I turned the interface off. The problem decreased but definitely did not stop.
If someone else is using direct reort import from Quest ?? are you having any problems with the interface?? I think the problem is in AC not the interface. I notice that the post from HIFM is interfaced with LabCorp not Quest.

I have the newest update as there were other glitches in the last. It is imperative that we all report the bug reports per my conversation yesterday with the VP. That is why she asked me to post this again on here. She even said she was going to be testing this in her practice. I am assuming we all have a test patient on file, if so import something you will have to sign and make comments. Then go to imported items and see if a) your signature is there and b) are the comments. You may be surprised.

Thank you for bringing this to our attention! We have been able to replicate the bug, and have determined the cause - and we agree completely that this could potentially lead to a signficant medicolegal issue if a provider is not able to prove they had handled the abnormal result. Here is our plan: 1. We will put out a new version (4.0.73) to fix the sign-off issue. We hope to have this released by the end of this week. 2. We will post a Critical Issue Announcement to all Amazing Charts users (which you should see when first logging into the program), and which will direct users to a webpage where the issue will be discussed. (There is also another potential critical issue with the Synchronization Utility which we are still investigating.) Again, thank your for alerting us and helping us replicate the bug. Jon

Deborah, thank you for that detailed reply. You are right that it is not fair to blame the MA, and I have not done that exactly. Our issue was different from this one, we have not been saving our lab messages to the imported items, but have been saving messages or addenda with regard to imported labs from LabCorp. After asigning the task of importing labs to a specific staff member and asking them to double check against the manifest we requested from LabCorp in the next suite we have been unable to pin down a specific error, and the errors have almost stopped.
I'll be sure to update my offices this weekend.

Just a quick update to this situation...
We have fixed the sign-off/comment bug discovered by HIFM (THANKS!!) and we have indeed discovered that there is a Synch utility bug as well. Both have been fixed, and we are now in the process of internally testing these fixes and the new installation package to ensure we've actually fixed the problems without introducing other issues.
We expect our internal testing to be completed by tomorrow afternoon and providing we don't find an issue, we expect to release the new build (V4.0.73) tomorrow evening.
