Our local hospital and a local nursing home both switched EMR's in the last few years. Hospital has local servers, I think - at least originally. Nursing home hosted in the heavenly cloud.
Both transitions were rough, little useful medical data got transferred.
And they had plenty of professional IT help, and apparently unlimited budgets.
