We have used a program called PDFredirect which will combine the sections into one document and even encrypt it. This is more straightforward. Then use a program call PDFfill to encrypt if it is going by email to be HIPPA compliant.
Both are free to use in professional environments but PDFredirect Professional is only $19.95 and PDFfill has a number of upgraded programs that are inexpensive but neither are obtrusive.
